Sheffield Wednesday fielded a young team that absorbed the late Leeds United pressure to stun the Premier League side.
Owls fans may have feared for the worst when they saw the line-up that Henrik Pedersen fielded against the top flight team – a majority of youth team players.
But they showed what they were made of with Leeds looking far off the pace in the first half at Hillsborough.
Indeed, it wasn't until the home side took the lead. Jamal Lowe put in a low cross that Karl Darlow attempted to intercept but instead turned into his own net.
After that howler from Darlow it was all Leeds with Daniel Farke bringing on the likes of Willy Gnonto and giving a debut to Dominic Calvert-Lewin.
Jayden Bogle put in a low drive through the legs of Ethan Horvath late on, with the goalkeeper playing on his home debut.
Horvath otherwise excelled, making big stops while Calvert-Lewin missed two notable chances late on.
After minutes of stoppage-time, the game went to penalties with Leeds missing all three of their spot kicks – two saves from Horvath and Calvert-Lewin blazing his over – while Wednesday kept their composure to secure a shock win.
Leeds United weren't the only Premier League team to be knocked out on penalties though with Huddersfield Town triumphing over Sunderland.
Elsewhere, Wolves took the lead against West Ham United before the Hammers fought back to make it 2-1.
Strand Larsen then struck in the space of two minutes late on to pile the misery on Graham Potter's side.
In another all-Premier League clash Brentford defeated Bournemouth 2-0 with the Cherries finishing the game with 10 men.
There was also big wins for Bradford City over Stoke City, Port Vale over Birmingham City, and Wrexham battled back to defeat Preston North End.
