Inter got the game’s first chance inside three minutes, with Marcus Thuram directing a flying header towards goal, but Mike Maignan kept his effort out.
Federico Dimarco then saw an effort deflected wide, and Petar Sucic blazed a rebound over the bar.
Milan had their first sight of goal after 15 minutes, when Davide Bartesaghi struck wide from the edge of the box.
Francesco Acerbi almost gave the hosts the lead before the half-hour mark with a header from a corner, but was denied by the woodwork.
Lautaro Martinez thought he had given Inter the opener with a one-time effort, only for Maignan to superbly tip his effort onto the post.
Inter’s Yann Sommer made no saves before the break but was almost beaten by Alexis Saelemaekers and Christian Pulisic late in the first half.
The hosts picked up from where they left off before the interval, putting pressure on Milan, but could not breach Maignan’s goal.
Milan broke the deadlock against the run of play through Christian Pulisic, who put the ball into the net after Sommer parried Saelemaekers’s shot into his path.
Hakan Calhanoglu squandered a big opportunity to restore parity for Inter, but Maignan brilliantly denied him from the spot.
The result takes Milan into second spot in the standings, two points adrift of table-toppers Roma, while Inter slip to fourth.
