Rafael Leao returns to the Milan bench, as Christian Pulisic, Santiago Gimenez, Adrien Rabiot and Luka Modric start, but decimated Napoli rely on Kevin De Bruyne, Scott McTominay and Rasmus Hojlund plus two debutants.

It kicks off at San Siro at 19.45 UK time (18.45 GMT).

This is a big clash between form teams and a potential first Scudetto battle for the reigning Champions of Italy.

Milan finally bring Rafael Leao back onto the bench for the first time in Serie A this season after his muscular injury in the Coppa Italia in August.

Mike Maignan also recovered, so only Ardon Jashari remains on the treatment table.

It’s an experienced midfield with Modric and Rabiot supporting Pulisic and Gimenez, leaving Ruben Loftus-Cheek on the bench with Christopher Nkunku.

They were in action midweek, crushing 10-man Lecce 3-0 in the Coppa Italia.

Napoli are decimated by injuries, as they made the trip without Romelu Lukaku, Leonardo Spinazzola, Mathias Olivera, Alessandro Buongiorno and Amir Rrahmani.

This forced an inevitable transformation, with Miguel Gutierrez given his season debut, having joined from Girona in August for €18m plus bonuses.

Luca Marianucci also gets his first start rather than Sam Beukema, with a totally revolutionised back four.

De Bruyne and McTominay support Hojlund in a more predictable front line.

Napoli are the only side still with a 100 per cent record in Serie A, but lost 2-0 to Manchester City in the Champions League after an early Giovanni Di Lorenzo red card.
