Napoli are on course to return to Serie A's summit after taking the lead with Inter.
The reigning champions broke the deadlock shortly after the half-hour mark when Kevin de Bruyne comfortably dispatched a penalty beyond Yann Sommer.
But the Belgium international's well-taken kick saw him forced off almost immediately as he appeared in visible discomfort while celebrating his strike.
Things went from bad to worse for De Bruyne as he had to be helped off the pitch just four minutes later as Mathías Olivera took his place for the Partenopei.
Antonio Conte will be praying the evergreen playmaker's issue is only minor.
