Martin Terrier was forced off injured shortly after coming on for Bayer Leverkusen at Heidenheim, the Bundesliga match finished 3-3 on matchday 27.
The French forward entered at half-time and was withdrawn in the 54th minute, less than 10 minutes later. According to L'Équipe, he fell while breaking forward, got up, then immediately asked to be replaced.
On the incident, the former Stade Rennais player appeared to have a problem with his right hamstring.
It comes a week after a heavy challenge from Nicolas Jackson on his right ankle before the interval during Bayern Munich against Bayer Leverkusen, from which he had escaped relatively unscathed.
Terrier, 29, had been invited to Stade Rennais' 125th anniversary on Sunday, and his presence is now uncertain.
