Ajax beat Utrecht on penalties after a 1-1 draw in extra time at Kras Stadion, with Maarten Paes saving twice in the shootout.
In the sixth minute of extra time, Davy Klaassen steered a left-footed finish to the bottom corner from Takehiro Tomiyasu's set piece to put the Godenzonen ahead.
Gjivai Zechiël levelled in the 16th minute of extra time with a low strike from outside the box.
In the shootout, Paes denied Sebastian Haller with the first kick and then saved Souffian El Karouani's effort to clinch it. Klaassen, Wout Weghorst, Mika Godts and Anton Gaaei all converted for the Godenzonen.
Vasilis Barkas kept Utrecht in the game over 120 minutes with seven saves. The Godenzonen carried the greater threat, finishing with 24 attempts and 63.1% possession. Utrecht defended deep under pressure, making 37 clearances and 11 interceptions.
